CHMA REGISTRATION PROCEDURES & POLICIES
These are the procedures and policies all of our students/students’ parents agree to during the registration process.
Tuition & Registration Payments
All monthly tuition payments will be processed automatically on the first of each month by a pre-authorized credit card, debit card, or auto bank draft. For example: We shall pre-charge your account on September 1st for all lessons occurring in September.
The cost of a private 30-minute music lesson is $35.00.
The cost of a private 60-minute music lesson is $70.00.
The cost of a private 60-minute Recording Artist Academy session is $85.00.
The cost of a private 60-minute Music Creation Lab is $85.00.
The cost of a private 60-minute recording session is $75.00.
I understand that tuition will not be pro-rated or refunded for cancellations and absences. A one-time registration fee of $25 will be charged per each student enrolled.
Cancellation & Absences
Consistency is huge when it comes to getting the most out of your lessons. It is my method to make sure that I create a system to ensure my child’s attendance (i.e. creating a recurring event with an alarm on my phone).
Tuition will not be prorated or refunded for cancellations. Each student, who misses a lesson for any reason, will be offered five private makeup lessons per year. Our calendar year starts September 1st and ends on August 31st. Unused makeup lessons will expire 6 months from the date of absence.
The only way we can continue offering private makeup lessons is to utilize the vacant slots from other student's cancellations. For this system to work, all students must give the office at least 24 hours notice prior to cancellation. Students that do not notify the office by phone, voicemail or email before their lesson will not be eligible for a makeup lesson. I understand that it is my responsibility to email the office to set up a makeup lesson at email@example.com.
If a CHMA music teacher must cancel a lesson, a substitute teacher in our system will be provided. If CHMA is unable to provide a substitute music teacher, a private makeup lesson will be scheduled at a future date.
Withdrawing From Lessons at CHMA
Should a student need to discontinue from a single lesson or CHMA entirely, I must fill out, sign, and submit a withdrawal form to our office 30 days in advance of withdrawal. No credits or refunds will be given for lessons missed prior to the withdrawal date.
Clarendon Hills Music Academy does not close during summer months. We are a year-round music academy. We encourage all of our students to continue lessons over the summer to maintain progress, prevent backsliding, and to ensure that they have a time slot the following fall. Vacation absences can be made up by attending one of your 5 makeup lessons given per year (September 1st - August 31st). Students that do not notify the office by phone, voicemail, or email 24 hours prior to cancellation, will not be eligible for a makeup lesson.
If you need to withdrawal for the summer, you will submit a withdrawal form 30 days before your withdrawal date. Please note, that we cannot hold a lesson time slot or guarantee the availability of a student's teacher in the fall for students that withdrawal for the summer.
Parent Involvement (Students K-8th Grade)
Parent involvement, especially for younger students, is really important to getting the most out of your lessons. We have an open door policy and welcome parents to sit in on the lessons. We do understand that parents are overwhelmed with other activities, so if you can't logistically sit in on the entire lesson, try to show up during the last 5 minutes to get an overview of what should be happening at home with practice.
We understand that high school students may think that the idea of their parents sitting in on the lesson does not encourage independence. In other words, they may feel it's super lame. In these situations, we encourage the student's parents to connect with their teacher via email just to see how things are going.
I understand that in order for my student to reach his/her potential, I must play a role in his musical development by supervising or guiding my child during practice and lesson time.
The undersigned gives permission to Clarendon Hills Music Academy, its owners, and operators to seek medical treatment for the participant or relatives of the participant in the event they are not able to reach a parent or guardian. The undersigned at this moment declares the participant to be in good physical and mental health. I allow Clarendon Hills Music Academy to call for medical transport to a local hospital in the event of a medical emergency.
Student information is kept private and confidential. Information may only be disclosed to faculty designated by the academy director. Clarendon Hills Music Academy may only breach the confidentiality agreement in the case of an extreme emergency or the case of evident child abuse or neglect.
By agreeing to this registration, I automatically opt into CHMA’s email list. Occasionally we will reach out to all of our active and non-active students via email. I will have the option to opt-out of our emails at any time. Please keep in mind that email is the only way we inform our students of scheduling, closures & other relevant information.
Video, Audio, and Image Waiver
I understand that my child/children may be photographed, videotaped, or recorded at the Clarendon Hills Music Academy facility or any Clarendon Hills Music Academy event. I give permission for photographs, video, and recordings of my child/children to be used for student albums, performance video series, and general social content. The same may also be used to promote Clarendon Hills Music Academy through press releases, brochures, and other on and offline promotional materials.
Student Safety & Injury Waiver
Clarendon Hills Music Academy is not responsible for children left unattended before or after the scheduled lesson. We are not liable for the safety of children who are unaccompanied when traveling to and from our location.
Participants expressly assume all risk and legal liability and waive and release all claims for injuries, damages, or loss sustained while participating in any and all activities connected with and associated with this program. I agree to assume the full risk of any and all injury, damage, hearing loss, or loss regardless of severity that my minor child or I sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child may have (or accrue to me or my minor child) as a result of participating in this program against the Clarendon Hills Music Academy, including its officials, agents, volunteers, contractors and employees (hereinafter collectively referred to as "Clarendon Hills Music Academy") I understand the nature of this program and have read and fully understand the above important information, warning of risk, assumption of risk, and waiver release of all claims.
Authorization Agreement for Auto Draft
I/We do hereby authorize Clarendon Hills Music Academy to establish recurring monthly tuition payments from my checking account, credit card, or debit card. These tuition payments are withdrawn on the first day of each month until an official Clarendon Hills Music Academy withdrawal form is submitted to our office 30 days before withdrawal date.
Revising Terms, Conditions, and Policies
To improve what we do, Clarendon Hills Music Academy reserves the right to improve and change our terms, conditions, and policies. If we do improve or change certain aspects of our policies, I will ALWAYS be informed by receiving a copy of our revised terms, conditions and policies yearly via email.
Private lessons - 30 or 60 minutes of one-on-one instruction to develop mastery of a musical instrument. No audition required. Lessons are offered in the following instruments:
Recording Artist Academy - Unique to CHMA, this program is designed for the more advanced musician. In this program, students will develop all of the necessary skills to record an album, write original material, and promote themselves through website building and social media engagement. A brief audition is required for enrollment.
Professional Recording Services - CHMA’s recording studio is fully equipped with state-of-the-art digital technology, current sound editing software. In addition, CHMA provides in-house producers to ensure professional quality sound production.
Live Performance - Twice a year, CHMA hosts shows to showcase students. These are open and free to all current CHMA students.
Tuition covers weekly lessons, student binder and free participation in CHMA shows.
Private Lessons: $35.00/30 min. ($70.00/60 min.)
Recording Artist Academy Program: $85/hour
Recording Services: $75/hour
We accept credit/debit cards or bank drafts. Payments are made and processed automatically at the beginning of each month, with an initial $25 registration fee per individual student. We do not offer refunds for missed lessons but allow five makeup lessons per year. In order to receive a makeup lesson, notice of cancellation must be given at least 24 hours in advance.

FREQUENTLY ASKED QUESTIONS
Question: What is your attendance policy?
Answer: Students are required to attend every scheduled lesson. Lessons occur every week with the exception of holidays (which are listed on our calendar).
Question: What if my student has to miss a lesson?
Answer: Five makeup lessons are provided per year, and are available as long as 24 hour notice is given. Please contact firstname.lastname@example.org as soon as you learn about a conflict or absence. For more information on makeup lessons, please see the Makeup FAQ section.
Question: Are there lessons during the summer?
Answer: Yes! CHMA runs both during the school year and throughout the summer. Most of our students enjoy lessons during the summer to continue progress. In addition, students will be guaranteed a time slot in the fall if they continue over the summer.
Question: What do you do on snow days?
Answer: CHMA typically runs in the afternoon, after snow conditions have cleared up. In the case that road conditions prevent teachers/students from arriving, we will notify you immediately and offer a makeup lesson. This will not count against your five makeups.
Question: What if my teacher is absent?
Answer: If a teacher cancels a lesson, we will try to find a substitute teacher for that day. If we cannot find a substitute teacher, we will schedule a private makeup lesson with your regular teacher at a later date. This will not count against your five makeup lessons.
Question: If I need to discontinue my lessons, what is the withdrawal process?
Answer: As outlined in our registration agreement, we need 30 days notice for all students that need to discontinue lessons on a permanent or temporary basis. Simply contact the office to let us know that you have to discontinue lessons. The office will provide you a withdrawal form to fill out. Due to our teachers' schedule, your time slot cannot be held during a break.
MAKE UP LESSON FAQ
Question: How many makeup lessons do I receive per year?
Answer: Each student is offered five private makeup lessons per year (September 1 - August 31st) assuming 24-hour notice is provided. Unused makeup lessons expire 6 months from the date of absence.
Question: How do makeup lessons work?
Answer: If you need to cancel a lesson, please let us know 24 hours in advance. Please do not contact your teacher directly about cancellation. Instead, text or call our office at 630-986-8742 or email service (at) chmusicacademy.com. We will let your teacher know that you are not coming. You can contact the office any time to schedule your makeup. Keep in mind that open slots change from week to week and your make up time will not necessarily sync to your regular lesson time. In many cases, your makeup lesson option might be on a different day or time frame than your normal lesson time. Unused makeup lessons expire 6 months from the date of absence.
Question: How much notice do I need to give in order to receive a makeup lesson?
Answer: We require 24 hours notice in order to receive a makeup lesson.
Question: Can I get a makeup lesson for a missed makeup lesson?
Answer: No, we cannot offer a makeup for the missed or cancelled makeup lesson. We recommend setting an alarm on your phone so you don’t forget.
MUSIC RELATED LESSON FAQ
Question: Who is the best person to contact about musical issues?
Answer: Your music teacher is the best person to contact about musical questions. If you need your teacher’s email, please contact the office at email@example.com and we will be happy to provide you with that information.
Question: What materials does my child need for lessons?
We provide a binder with sheet music, practice planners, and other informational materials. A smart phone/laptop with speakers/internet connection is very helpful for our students, because our teachers customize back-up tracks for each student. We will provide suggestions for instruments depending on your students’ need.
Question: Can I sit in and watch my child’s lesson?
Answer: Always! We encourage parents to sit in on any and all lessons. Parent involvement, especially for younger students, is imperative for musical growth. We have an open door policy and welcome parents to sit in on any part of your child's lesson. If you are unable to sit in on the entire lesson, please try to observe the last 5 minutes to get an overview of what the student should be practicing at home.
Question: How else can I help my student?
Answer: Your students' music binder is the best resource to check weekly goals and additional notes from your teacher. You can also help out by starting tracks for students, clapping out the beats, or just lending an ear while your student practices.
Question: How much time should my child or I spend practicing at home?
Answer: This depends on the student’s age and playing level, but we suggest a minimum of 15 minutes a day to 30-45 minutes daily. Each teacher’s philosophy can differ slightly on this subject. Make sure to consistently connect with your teacher about practicing goals.
Question: Are there any performance opportunities for our students?
Answer: We offer two optional performance opportunities each year, one in late fall and one in late spring. There is no charge for these performances. We typically conduct a free will offering to raise funds for a worthy cause.
