CANCEL MY LESSON FORM
IMPORTANT: if you are canceling multiple students on different time slots, you must submit the "Cancel My Lesson" form for each student and lesson time slot. One student per instance.
By simply filling out this form, your teacher and the office will be notified instantly. You do not have to send additional texts to your teacher or office. One and done!
PLEASE NOTE: This "Cancel My Lesson" form has to be submitted before scheduling your make up lesson using the Parent Portal. It can take up to 24 hours to get each cancellation approved for a make up lesson opportunity.
